Home Health Care Insurance Broker in Fort Worth, TX
If you or a parent may need care at home, who pays for the help Medicare leaves out? Medicare covers some skilled home health care, but not all daily help. Wilkerson Insurance Agency is an independent broker that compares home health care insurance options for Fort Worth families.
Your Home Health Care Insurance Broker in Fort Worth
Home health care insurance is not one product. It is a group of options that may help pay for care at home, from recovery support after surgery to long-term care planning. The right mix depends on the coverage you already have. A home health care insurance broker compares those options and explains how they work with Medicare or your health plan. Because we are independent, you see more than one carrier side by side. For a statewide overview, see our home healthcare plans page.
For every Fort Worth family, we:
- Review your Medicare, health plan, and employer benefits for home care coverage.
- Explain which care Medicare may cover and which care it leaves out.
- Compare private options, such as recovery plans, hospital indemnity, and long-term care insurance, where carriers offer them.
- Point you to Texas Medicaid and local aging services when they may fit your situation better than private coverage.
- Help with the application and the health underwriting questions carriers ask.
- Stay available for questions and guidance after you enroll.
Home Health Care and Home Care: Two Different Types of Help
People often use "home health care" and "home care" to mean the same thing, but they are very different in what they provide, who delivers them, and who pays. Medicare and most health insurance plans treat them as separate categories:
| Type of Care | Services Included | Medicare Coverage |
|---|---|---|
| Skilled home health care | Nursing visits, wound care, injections, and physical, occupational, or speech therapy | May cover when you meet specific clinical conditions |
| Personal care | Help with bathing, dressing, grooming, eating, and using the bathroom (ADLs) | Not covered when it is the only care you need |
| Homemaker and meal services | Shopping, cleaning, laundry, cooking, and meals delivered to your home | Not covered |
| Round-the-clock care | Custodial care at home 24 hours a day | Not covered |
Personal care and homemaker help are where catastrophic out-of-pocket coverage gaps tend to appear. The next sections show what Medicare pays for and where private options fit.
Medicare Coverage for Home Health Care
Original Medicare covers medically necessary skilled care at home only when you meet strict qualifying conditions, according to Medicare.gov:
- A doctor or other allowed provider sees you in person, confirms you need home health care, and orders it.
- You need part-time or intermittent skilled services, such as skilled nursing or therapy.
- You are homebound, which means leaving home is not recommended because of your condition, or you have trouble leaving without help.
- You are normally unable to leave home, and leaving takes a major effort.
- A Medicare-certified home health agency provides the care.
Care is usually part-time. Skilled nursing and aide care combined generally total up to 8 hours a day and 28 hours a week (or up to 35 hours a week for a short time if medically necessary). A Medicare Advantage plan must cover what Original Medicare covers, but it may require you to use agencies in its network. A GAO review found that only about 17 percent of plans offered in-home support services, such as help with dressing and housework. Check your plan's benefits before you count on them, and see our guide to Medigap vs Medicare Advantage for the differences.
The Coverage Gap After a Hospital Stay in Fort Worth
After a hospital stay, a doctor may order home health care, and you have a say in which agency provides it. Fort Worth hospitals include Baylor Scott & White All Saints Medical Center, Texas Health Harris Methodist Hospital Fort Worth, Texas Health Harris Methodist Hospital Southwest, and JPS Health Network. Patients leaving any of them may receive skilled nursing or therapy visits at home.
Medicare may cover those nursing and therapy visits. It does not cover personal care when that is the only care needed, nor does it pay for someone to sit with you, help you bathe, prepare meals, or stay overnight once skilled therapy ends. That is where family caregivers step in or private-pay caregiver hours begin.
Sources of Payment for Care at Home
No single source pays for every kind of home care. You may need to combine more than one, and the right mix depends on the care needed, your health, and your budget:
| Source | What It May Pay For | Limit to Know |
|---|---|---|
| Original Medicare | Part-time skilled nursing, therapy, and aide help tied to skilled care | Homebound rule and certified agency required; no standalone personal care |
| Medicare Advantage | Everything Original Medicare covers, plus any extra benefits the plan offers | Network rules apply, and extra in-home benefits vary widely by plan |
| Medigap (Medicare Supplement) | Your share of Medicare costs, such as copayments, coinsurance, and deductibles | Generally does not cover long-term custodial care or private-duty nursing |
| Employer or individual health plan | May cover some skilled home health care, depending on the policy | Rules, limits, and pre-authorizations vary; check the summary of benefits |
| Recovery or indemnity plan | A set benefit during recovery, depending on the policy | Availability, waiting periods, and exclusions vary by carrier, age, and health |
| Hospital indemnity | A fixed cash amount for the hospital events a policy lists | Not a full care plan; pays only for listed triggering events |
| Long-term care insurance | A daily or monthly benefit for care at home or in a facility | Underwriting required; benefits trigger when 2+ ADLs cannot be performed |
| Texas Medicaid (STAR+PLUS / Community) | Personal care and home support for qualifying low-income individuals | Strict income, asset, and functional need qualifications apply |
| Savings or HSA funds | Qualified care costs you pay directly | An HSA is not insurance, and only qualified medical expenses are tax-free |
Because every source has limits, we start with a gap review: what you have, what it pays for, and what is left. Our guide on how Medigap fills gaps shows where Medicare Supplement plans help and where they stop.
Private Coverage Options That May Help
Private options fill different gaps. Availability, benefits, and prices vary by carrier, age, health, and location, so we confirm what each carrier offers before we quote:
• Designed for recovery immediately following surgery, illness, or hospital stays.
• Benefits paid as set cash stipends directly to you or your caregivers.
• Shorter coverage durations (typically 12 months or less per occurrence).
• Simpler health questions with faster underwriting turnaround.
• Comprehensive coverage for extended chronic illness, frailty, or cognitive impairment.
• Triggered by inability to perform 2 of 6 Activities of Daily Living (ADLs) for 90 days.
• Daily or monthly benefit pools covering licensed home caregivers, assisted living, or memory care.
• Texas Long-Term Care Partnership Program protection against Medicaid spend-down.
In addition, hospital indemnity insurance pays a fixed cash amount for the hospital events a policy lists. The cash may be used toward recovery costs at home, such as caregiver hours, meal delivery, or utility bills while recuperating.
Home Care Costs in Fort Worth
Knowing the cost of care helps you decide how much protection to look at. CareScout's 2025 Cost of Care Survey reports the Texas median costs below. Rates in Fort Worth can differ, so treat them as estimated planning figures:
| Type of Care | Texas Median Rate (2025) | Note |
|---|---|---|
| Non-medical caregiver | $30 per hour | Personal care, cooking, cleaning, and errands at home |
| Home health aide (agency) | $33 per hour | Assisting with personal care and basic health monitoring |
| Private duty registered nurse | $75 to $95 per hour | Skilled nursing visits, complex wound care, and IV medications |
Hours add up quickly. The table below multiplies the $30 median hourly rate by weekly hours to show estimated monthly and annual family costs:
| Hours of Help per Week | Estimated Monthly Cost | Estimated Yearly Cost |
|---|---|---|
| 10 hours / week | $1,300 | $15,600 |
| 20 hours / week | $2,600 | $31,200 |
| 30 hours / week | $3,900 | $46,800 |
| 44 hours / week (Full-time) | $5,720 | $68,640 |
The 44-hour row matches the survey's yearly median of $68,640 for a non-medical caregiver. Medicare does not cover this kind of care when it is the only care you need. For comparison, at 44 hours a week the estimated cost of home care is close to the Texas median for assisted living, while at 20 hours a week it is less than half of it.
How Policy Choices Shape Premium Costs
For recovery plans, hospital indemnity, and long-term care insurance, premiums depend on your age, health status, and policy design choices:
| Policy Feature | Higher Premium | Lower Premium |
|---|---|---|
| Monthly benefit | Higher ($4,000 to $6,000+) | Lower ($1,500 to $2,500) |
| Waiting / elimination period | Shorter (0 to 30 days) | Longer (90 to 100 days) |
| Benefit duration | Longer (3 to 5 years or lifetime) | Shorter (1 to 2 years) |
| Inflation protection | Included (3% to 5% compound) | Not included |
NAIC notes that adding home care benefits to a long-term care policy generally raises its cost, but inflation protection remains one of the most critical safeguards against rising healthcare costs. Our home health care insurance cost guide covers premium factors in more detail.
Using an HSA or Tax Rules to Help Pay
The IRS says HSA funds used for qualified medical expenses are 100% tax-free. Based on IRS Publication 502, nursing services you pay for—including help such as administering medication, changing dressings, and bathing or grooming connected to a patient's medical condition—can count as tax-deductible medical expenses. Qualified long-term care services for a chronically ill individual may also qualify. For account rules and compatible plans, see our Fort Worth HSA plans page.
Free Help in Tarrant County Before You Buy
You do not have to buy insurance to understand your community options. Tarrant County provides free, impartial public resources:
- Area Agency on Aging of Tarrant County: Part of United Way of Tarrant County, offers free healthcare benefit counseling on Medicare, Medicaid, and long-term care options.

- Ask what specific care the policy pays for, and whether it requires a licensed home health agency or registered nurse.
- Confirm what clinical events must happen before benefit payments start (e.g., prior hospital stay vs. ADL triggers).
- Find out the exact elimination period (waiting period) and how many months or years benefits will last.
- Check whether the benefit is paid daily, weekly, or monthly, and whether checks go directly to you or to the care agency.
- Verify whether the policy allows benefits to increase over time with inflation protection.
- Review exclusions for pre-existing medical conditions and understand whether premiums can change in the future.
