Your Group Health Insurance Broker in Fort Worth

A group health insurance broker compares plans from several carriers, models what each choice costs you and your employees, and handles the paperwork. Because we are independent, you see more than one carrier's plans side by side. We also explain the Texas rules that shape what you can buy, so nothing surprises you at underwriting.

Fort Worth employers range from one-owner shops hiring a first employee to firms with hundreds of workers. Census data shows 33,895 employer firms in Tarrant County in 2023, and the Dallas Regional Chamber reports that 76% of establishments in the DFW region have fewer than 100 employees.

For a statewide overview, see our group health insurance plans page. For every Fort Worth employer, we:

  • Review your headcount, employee ages, ZIP codes, and benefit goals.
  • Compare group medical plans from the carriers available to your group.
  • Model employer contribution options before you commit.
  • Check the Texas participation and contribution rules for your group.
  • Manage the application, underwriting follow-up, and employee enrollment.
  • Stay available for new hires, renewals, and questions.

Texas Small Group Rules for Fort Worth Employers

The Texas Department of Insurance sets the framework for small employers. These are the rules that matter most when you shop.

Rule What it means
Small employer A business with two to 50 employees, regardless of how many hours they work
Who you must offer If you offer a plan, you must offer it to employees who work 30 or more hours a week, plus their dependents
Owner coverage Owners can enroll if at least one employee also enrolls
Employer contribution The law does not require you to pay premiums, but an insurer may require at least 50% of your employees' premiums
Participation Most insurers require at least 75% of full-time employees to join. Employees with other coverage do not count, and a group with only two eligible employees needs 100% participation
Buying window You can buy at any time of year. If you miss the participation rule, the insurer can make you wait for open enrollment, November 1 to January 15
New hires New employees get at least 31 days from their start date to enroll, and you can require a waiting period of up to 90 days
Pre-existing conditions Insurers cannot deny or limit coverage for them, and your employees' health cannot raise your rates

ACA and COBRA Rules for Fort Worth Employers

  • Under 50 full-time equivalent employees: Texas law does not require small employers to offer health insurance, and the federal employer mandate starts at 50 full-time equivalent employees.
  • 50 or more full-time equivalent employees: Applicable Large Employers must offer coverage that meets minimum value and affordability standards, or they may face IRS penalties.
  • Employees who leave: Federal COBRA applies to employers with 20 or more employees, and the Texas continuation law applies to employers of any size. Former employees who keep coverage pay the full cost.

This page gives general information and is not legal or tax advice. An attorney or CPA can confirm how these rules apply to your business.

Group Health Plan Structures Available to Fort Worth Employers

Structure How it works Often fits
Fully insured You pay a fixed monthly premium, and the carrier carries the claims risk Small groups that want predictable costs
Level-funded You pay a set monthly amount for projected claims, administration, and stop loss, with a possible refund if claims run low Stable, generally healthy teams
Self-funded with stop loss You pay claims directly and buy stop loss coverage for very large claims Larger employers with administrative support
ICHRA You provide a monthly allowance that employees use toward individual plans Teams spread across counties, or with changing headcount

Within each structure, you choose a plan type such as PPO, HMO, EPO, or a high-deductible plan paired with HSAs. Read our level-funded guide, our comparison of ICHRA vs traditional group, or our article on self-funded vs fully insured coverage for more detail. Our HSA plans page explains how high-deductible plans work.

Fort Worth workers can live in several counties, so we check network access where your employees live and get care.

Group Health Insurance Costs for Fort Worth Employers

KFF's 2025 Employer Health Benefits Survey found that firms with 10 to 199 workers paid an average of $9,211 a year for single coverage and $26,054 for family coverage. Covered workers at those firms contributed about 16% of the single premium on average, and 29% were in plans where the employer paid the entire single premium.

Your own price depends on your group. Insurers consider the age of your employees, tobacco use, your location, the plan type, and the network. The premium for a 64-year-old is three times the premium for a 21-year-old, and tobacco use can add up to 50%, spread across the group.

Higher deductibles usually lower premiums but raise what employees pay out of pocket. We do not quote a price on this page, because your group sets it.

Request a quote with your employee census, and we show estimated monthly costs and contribution options from several carriers. Our Texas group costs guide explains how premiums are built.

Tax Tools for Fort Worth Employers

Small Business Health Care Tax Credit

Your business may qualify for the federal credit if it:

  • Has fewer than 25 full-time equivalent employees.
  • Pays average wages below an inflation-adjusted IRS limit.
  • Pays at least 50% of the cost of employee-only coverage.
  • Buys a qualified plan through the SHOP Marketplace, or meets a limited exception.

The credit can cover up to 50% of premiums for small businesses and 35% for small tax-exempt employers, for two consecutive years. It works on a sliding scale, so smaller employers get larger credits.

It is not automatic, and a tax professional should confirm your numbers. Our small business tax credit guide covers the steps.

Section 125 Premium-Only Plans

A Section 125 plan lets employees pay their share of premiums with pre-tax dollars, which can lower taxable wages and certain payroll taxes. It requires a written plan document and yearly administration. Ask your tax professional how it applies to you, and ask us what the paperwork involves.

Group Benefits Beyond Medical

Medical coverage is the core of a benefits package, and other benefits can make it stronger. We compare these options too. For help with other coverage, visit our Fort Worth health insurance broker page.

Fort Worth Employers and Industries

Fort Worth's economy mixes aviation and aerospace, healthcare, logistics, manufacturing, and the military. Major employers include American Airlines, Lockheed Martin, JPS Health Network, Cook Children's, Texas Health Harris Methodist Fort Worth, and Naval Air Station Fort Worth Joint Reserve Base. On the north side, the AllianceTexas logistics hub sits near I-35W and State Highway 170 and hosts distribution and manufacturing employers.

Fort Worth employers in these industries can have workers who live across Tarrant and nearby counties, so network access matters.

Smaller businesses make up much of the picture. Census data counts 240,907 nonemployer businesses in Tarrant County in 2024, which means owners who work without paid staff. If you are hiring your first employee, ask us how the Texas two-employee minimum applies to your business.

Hospital access matters to employees. Networks differ by carrier and plan, so we verify that the hospitals and specialists your employees use are in the network before we recommend a plan.

Common Group Health Mistakes Fort Worth Employers Make

Choosing by Premium Alone

A low premium can come with a high deductible that your employees feel every time they need care.

Missing the Participation Rule

If fewer than 75% of eligible full-time employees enroll, the insurer can make you wait until open enrollment.

Ignoring Employee ZIP Codes

A network that works near your office can be thin where your employees live.

Miscounting Employees

Full-time equivalent counts decide your ACA obligations and tax credit eligibility, so count with care.

Setting Your Contribution Without Modeling It

Insurers may require at least 50%, so compare contribution levels against your budget before you commit.

Frequently Asked Questions: Fort Worth Group Health Insurance
What Are the Rules for Small Group Health Insurance in Texas?+
In Texas, a small employer is defined as having 2 to 50 eligible employees. Under Texas law, carriers cannot deny coverage to a small group based on the health status of employees. Typically, carriers require at least 50% to 75% of non-waiving full-time employees to participate and the employer to contribute at least 50% of the employee-only premium.
Can Fort Worth Employers Offer Level-Funded Health Plans?+
Yes. Level-funded plans are increasingly popular for small and mid-sized Fort Worth employers with relatively healthy workforces. You pay a set monthly fee that covers administrative costs, stop-loss insurance, and a claims pool. If your claims are lower than expected, your business receives a refund or credit at renewal.
What Is the Minimum Employer Contribution Required in Texas?+
Most carriers require the employer to contribute at least 50% of the employee-only premium for the base plan. Employers can choose to contribute higher percentages or contribute toward dependent coverage to make benefits more attractive.
Does My Small Business Have to Offer Health Insurance Under ACA?+
Under the Affordable Care Act, businesses with fewer than 50 full-time equivalent (FTE) employees are not penalized if they choose not to offer coverage. However, offering health coverage remains one of the most effective tools for hiring and retaining skilled workers in the competitive DFW job market.
Do You Charge a Broker Fee to Fort Worth Employers?+
No. Our group health consulting, plan design, annual renewal reviews, and employee enrollment services come at no charge to your company. Carrier commissions are already built into the standard premiums.
